A glass of wine with dinner, a few drinks on a night out, or a longer evening that turns into a late one: alcohol is woven into a lot of social life, and most of us know it leaves us dehydrated the next day.
What is less well known is that alcohol reaches your eyes directly, altering the tear film that keeps them comfortable, clear and protected. If you have ever woken up after drinking with eyes that feel gritty, dry or oddly blurred, that is not a coincidence.
At Safarian and Simon, we are asked about this more often than you might expect during a dry eye assessment, usually by patients who already manage some level of dry eye and have noticed it gets worse after drinking.
Does alcohol cause dry eyes?
Yes, and the research on this has grown substantially in recent years. A large population-based study of 77,145 participants from the Dutch Lifelines cohort found that 30 percent overall had symptomatic dry eye.
A separate meta-analysis pooling 10 studies from 8 articles, published in the International Journal of Ophthalmology, concluded that alcohol consumption significantly increases the risk of dry eye syndrome, independent of smoking, hypertension, diabetes and thyroid disease history.
The mechanism is now well understood at a chemical level. Controlled studies measuring the effect of a single alcohol intake found that alcohol increases tear film osmolarity, shortens tear film breakup time, and increases ocular pain within hours of drinking. Alcohol was still detectable in the tear film eight hours after intake, at concentrations close to half the level found in the blood at the same time.
How alcohol affects your tear film
Your tear film is a three-layer structure: a mucus layer that anchors the tears to the eye, a watery layer that hydrates the surface, and an oily outer layer that stops the whole film evaporating too quickly. Alcohol disrupts more than one of these layers at once, which is why its effect on eye comfort is more pronounced than dehydration from, say, missing a few glasses of water.
- Dehydration reduces the water available for tear production. Alcohol is a diuretic, so it increases fluid loss through urine, and the eyes are one of the first tissues to feel that shortfall.
- Altered tear chemistry makes tears less stable and more prone to breaking up across the eye’s surface before the next blink, the effect measured directly as reduced tear film breakup time in the research above.
- Increased inflammation from alcohol consumption can affect the meibomian glands in the eyelids, which produce the oily layer that prevents evaporation. Disrupted glands mean faster evaporation and more evaporative dry eye.
- Nutrient depletion, particularly of vitamin A and omega-3 fatty acids, compounds the problem over time. Both nutrients are directly involved in tear production and corneal surface health.
Blurred vision, bloodshot eyes and night vision
Alcohol’s effect on the eyes is not limited to dryness. Blurred vision and bloodshot eyes are both common after drinking, the former linked to the disrupted tear film distorting light as it enters the eye, the latter to alcohol dilating blood vessels across the ocular surface.
Night vision is a less commonly discussed but genuinely significant effect. Research using objective optical quality measurement after alcohol consumption found measurable increases in intraocular light scattering, with deterioration continuing over time after drinking.
A related driving performance study published in Scientific Reports found that alcohol consumption significantly worsened both contrast sensitivity and retinal straylight, the technical term for the light scatter that produces glare and halos, alongside a measurably impaired ability to drive.
If you have ever found headlights unusually dazzling or streetlights unusually halo-ringed after a drink, this is very likely why, and it is a further reason not to drive.
Does alcohol affect men and women’s eyes differently?
This is one of the more striking findings in recent research, and one worth knowing if you are trying to work out why a partner or friend seems far more affected than you are. The same Dutch Lifelines cohort study found a clear sex-specific effect: alcohol use significantly increased the risk of symptomatic dry eye in women, but not in men.
In men, increasing alcohol intake was actually associated with a mild protective effect against symptomatic dry eye, a finding the researchers themselves cautioned should be interpreted carefully, given alcohol’s other, better-established health risks. The exact biological reason for the sex difference is still being investigated, but it is a genuine, measured effect rather than anecdotal.
Chronic alcohol use and long-term effects on the eyes
Occasional drinking tends to produce short-term symptoms that resolve within a day or two as the body rehydrates.
Chronic alcohol use is different. Consistent nutrient depletion, ongoing inflammation and repeated tear film disruption can contribute to a more established dry eye disease, one that does not simply reverse the morning after you stop drinking.
Alcohol withdrawal itself can also disrupt tear production, since it affects the nervous system signalling involved in tear film regulation, so the picture is not always a straightforward case of “drink less, dry eye improves immediately.” If dry eye has become a persistent, rather than occasional, problem, it is worth having it properly assessed rather than assumed.
How to prevent dry eyes from alcohol
You do not have to give up drinking to protect your eyes, but a few habits make a measurable difference:
- Hydrate before, during and after. A glass of water for every alcoholic drink is the simplest and most effective single habit, since it directly counters the dehydration eyes experience first.
- Use preservative-free eye drops before drinking, during a longer evening, and again the next morning. This is the same advice we give frequent flyers with dry eye issues, whose eyes face a similar dehydration challenge in the air.
- Support your diet with vitamin A and omega-3s, particularly if you drink regularly, to offset the nutrient depletion that compounds dry eye over time.
- Give your eyes a proper night’s rest after drinking. Tear production naturally reduces during sleep, so a dehydrated start makes overnight dryness worse.
When it is more than a hangover
Occasional dryness after a night out is common and usually resolves on its own. But if your eyes often feel gritty, red or uncomfortable, even without alcohol, that is usually a sign of an underlying dry eye condition.
This is worth taking seriously: many people notice they suffer from these symptoms, which suggests a great many people are living with a manageable, treatable condition without realising there is a name for what they are feeling.
